Three things decide which number applies to you: the region (Palm Beach/Broward/Dade, Monroe, or everywhere else), the staffing ratio, and whether you bill as a solo provider or an agency (two or more employees, including the owner).

The thresholds that change your code
The table isn't just rates — it encodes billing rules. Life Skills Development 3 (ADT) and 4 (Prevocational) switch to the monthly rate at 80+ attendance hours in a month; Residential Habilitation switches monthly at 24+ days in a month. All Life Skills Development combined is capped at 56 hours per week. Personal Supports has daily quarter-hour maximums, Respite's quarter-hour and day codes are for individuals under 21, and Specialized Mental Health Counseling is limited to 8 quarter hours per week. Billing an expired or exhausted service authorization is still the fastest way to a denial, whatever the rate.

What's the difference between the solo rate and the agency rate?
Solo rates apply to an individual practitioner operating alone; agency rates apply to providers with two or more employees, including the owner. Agency rates are higher to reflect overhead — for example, Personal Supports 1:1 pays $5.69 per quarter hour solo and $6.07 agency in most counties.
Which counties get the 'geographical' rate?
