APD's paperwork is scattered across the agency's site, the Florida Administrative Code, and Qlarant — and search results are littered with third-party “fillable” copies of outdated versions. This index sticks to what each document is, when it's due, and the official home of the current version.

Incident & medication forms (the ones with clocks)

Incident reporting — critical incidents within 1 hour, reportable incidents within 1 business day, follow-up within 5 business days; our incident reporting guide covers categories and workflow. Medication Error Report — Form 65G-7.006A — submitted within 24 hours; an electronic or agency-generated version capturing the same information is acceptable under the rule. The MAR — Form 65G-7.008A — kept current per administration; pharmacy-generated and electronic MARs are allowed. Details in the medication error guide.

Service documentation (mostly form-free)

There is no standardized service-log form — the iBudget Handbook defines required contents, and any format carrying them works (see service log vs. progress note). Residential and day services add daily attendance logs, quarterly or monthly summaries (the ninth monthly / third quarterly serves as the annual report), and implementation plans. Person-Centered Support Plans are the notable iConnect-form exception for support coordinators.

Licensing & enrollment forms

Since the 65G-2 rewrite took effect (March 26, 2026), facility licensing runs on the License Application Form 65G-2.002-A (December 2025) and the APD Licensing Disciplinary Chart Form 0041 — both incorporated by reference in the rule and hosted on flrules.org. Renewals are due 45+ days before expiration. Expanding into new services or regions goes through the expansion request process under Rule 65G-4.0215, and enrollment itself is covered in our become-a-provider guide. Staff-side: Level 2 screening through the AHCA Clearinghouse, the APD Attestation of Good Moral Character, and the roster requirement live in the employee file checklist.

Review & remediation paperwork

After a PDR: Qlarant's Reconsideration Request Form (billing-discrepancy findings only, within 30 days of the report mailing, fax submission) and the Plan of Remediation to your Regional APD office. For licensed facilities, the 65G-2.004 Notice of Noncompliance carries a 15-calendar-day Corrective Action Plan deadline.

Most of this paperwork is really a filing discipline. APDHQ's checklists track every required document per consumer and employee — with expirations — so the form you need in an hour-one incident or a 7-day desk review is already where it belongs.

Frequently asked questions

Where do official APD forms live?

Three places: apd.myflorida.com (provider forms, advisories, iConnect job aids), flrules.org (forms incorporated by reference into rules, like the licensing application and disciplinary chart), and florida.qlarant.com (review checklists and the reconsideration request form). Beware third-party 'fillable form' sites — they often serve outdated versions.

Can we use our own versions of APD forms?

Often, yes. Rule 65G-7 accepts an electronic or agency-generated Medication Error Report capturing the same information as Form 65G-7.006A; there is no standardized service-log form at all; and under APD's grace-period advisory (2026-016, effective July 1, 2026), Qlarant accepts most documentation in any format. The notable exceptions run the other way for QOs: Person-Centered Support Plans use the iConnect forms.

Which deadlines matter most across these documents?

The ones with clocks: critical incidents reported within 1 hour and reportable incidents within 1 business day (with the 5-day follow-up); medication error reports within 24 hours; a Corrective Action Plan within 15 calendar days of a licensing Notice of Noncompliance; Qlarant reconsideration requests within 30 days of the PDR report mailing; license renewals filed 45+ days before expiration.
